Registration Information
Completing this form allows players to join our league as spare skaters and goalies and will be part of our waiting list should full time spots open up. There is a $30 cost to join as a spare to cover insurance and administrative expenses. All other costs are for games played. Each Game is $20 for skaters. There is no charge per game for Spare Goalies. With the $30 Administration Fee, each spare skater will get their first game of the season for free. All new players to the league will be required to show proof of age.

Spares Fees
There is a $30 cost to join as a spare to cover insurance and administrative expenses. All other costs are for games played. Each Game is $20 for skaters.     By supplying my email address, I agree to receive registration, league, game and general information from Caledonia Oldtimers Hockey and it's affiliated Associations.  The email address(es) provided above will be used for member communication by Caledonia Oldtimers Hockey and Team Reps throughout the hockey season.  

    Please notify the Caledonia Oldtimers Hockey Executive via the Caledonia Oldtimers Hockey website to have an email id removed from the mail list.
  2. Over the course of the season, player names (including team lists) are included on the Caledonia Oldtimers Hockey Association website, public Caledonia Oldtimers Hockey displays and possibly newspaper publications. Additionally, throughout the season, individual and team pictures and videos are taken at various Caledonia Oldtimers Hockey Association events (and may be used beyond the current year). Your permission is requested to use your full name and photographs/videos .
  3. RELEASE AND DISCHARGE (please read carefully):

    I hereby release and forever discharge Caledonia Oldtimers Hockey Association, its Officers, Directors, Convenors, Team Reps, Referees or other Officials and the Town of Caledonia (County of Haldimand) from all claims, demands, damages, actions or causes of action arising or to arise by and reason because of my participation in any Caledonia Oldtimers Hockey program, in this or any successive years(s), including (but without limiting the generality of the foregoing) any and all dental and medical bills and further of and from all claims or demands whatsoever in law or in equity which I, my heirs, executors, administrators or assignors can, shall or may have by reason aforesaid.
